Charles Flahault, 34090 Montpellier en salle des actes<br \/><strong>En visioconf\u00e9rence<\/strong> : https:\/\/umontpellier-fr.zoom.us\/j\/94572729784?pwd=e895b22WBGQETzFb2N0nZUytkT6SKn.1<\/p><p><strong>R\u00e9sum\u00e9 de la th\u00e8se<\/strong><br \/>L\u2019antibior\u00e9sistance menace la sant\u00e9 publique mondiale. Parmi les r\u00e9sistances les plus pr\u00e9occupantes, celles associ\u00e9es aux carbap\u00e9n\u00e8mes, antibiotiques de dernier recours en clinique, sont consid\u00e9r\u00e9es comme \u00e9mergentes en France. La dynamique environnementale de ces r\u00e9sistances reste encore largement m\u00e9connue. Cette th\u00e8se explore, \u00e0 l\u2019\u00e9chelle du territoire de la m\u00e9tropole de Montpellier, la contribution d\u2019un hydrosyst\u00e8me dans la dynamique de la r\u00e9sistance aux carbap\u00e9n\u00e8mes.<br \/>L\u2019exploration environnementale de la r\u00e9sistance aux carbap\u00e9n\u00e8mes a \u00e9t\u00e9 r\u00e9alis\u00e9e dans quatre compartiments hydriques contrast\u00e9s : deux eaux douces (la source du Lez, principale ressource en eaux potables de la m\u00e9tropole, et le Verdanson, un cours d\u2019eau urbain) et deux eaux us\u00e9es (municipales et hospitali\u00e8res). Le recouvrement important des communaut\u00e9s mais aussi la r\u00e9sistance aux carbap\u00e9n\u00e8mes d\u00e9tect\u00e9e dans toutes les eaux, y compris celles soumises \u00e0 peu de pressions anthropog\u00e9niques et la pr\u00e9sence r\u00e9currente de taxons potentiellement navettes entre l\u2019environnement et population humaine sugg\u00e8rent l\u2019existence d\u2019un cycle environnemental de la r\u00e9sistance aux carbap\u00e9n\u00e8mes malgr\u00e9 son haut niveau d\u2019hygi\u00e8ne collective au sein du territoire \u00e9tudi\u00e9. <br \/>Par ailleurs, la surveillance de l\u2019antibior\u00e9sistance dans les eaux us\u00e9es hospitali\u00e8res de g\u00e8nes de r\u00e9sistance \u00e9mergents (vanA, blaOXA-48, blaNDM et blaKPC) par qPCR et dPCR s\u2019est r\u00e9v\u00e9l\u00e9e faisable en conditions r\u00e9elles, et coh\u00e9rente avec l\u2019\u00e9pid\u00e9miologie clinique connue pour blaNDM. Toutefois, la d\u00e9tection constante \u00e0 bas bruit de vanA, et forte abondance des g\u00e8nes blaOXA-48 et blaKPC dans les eaux us\u00e9es hospitali\u00e8res, sans corr\u00e9lation avec le portage connu chez les patients, r\u00e9v\u00e8le un d\u00e9calage entre circulation clinique et environnementale. De m\u00eame, les esp\u00e8ces cultivables productrices de carbap\u00e9n\u00e8mases dans les effluents (principalement Citrobacter spp.) ne correspondent pas \u00e0 celles retrouv\u00e9es majoritairement retrouv\u00e9es chez les patients, renfor\u00e7ant l\u2019hypoth\u00e8se d\u2019une dynamique de la r\u00e9sistance aux carbap\u00e9n\u00e8mes propre aux r\u00e9seaux d\u2019eaux us\u00e9es. Les isolats du genre Citrobacter, identifi\u00e9 comme navette potentielle de la r\u00e9sistance aux carbap\u00e9n\u00e8mes, portent fr\u00e9quemment des g\u00e8nes de carbap\u00e9n\u00e9mase dont blaKPC dans les divers compartiments hydriques de l\u2019\u00e9tude.
