Ce projet vise \u00e0 identifier le risque de transmission du paludisme en Tha\u00eflande, dans la province de Sisaket qui borde le Cambodge et qui est historiquement reconnue comme une zone \u00e0 forte pr\u00e9valence de paludisme.<\/p>\n<p>La th\u00e8se comporte plusieurs volets : i) une analyse bibliographique de la transmission du paludisme et de son \u00e9volution apr\u00e8s la mise en \u0153uvre de strat\u00e9gies de contr\u00f4le et d&rsquo;\u00e9limination en Tha\u00eflande et au niveau local, ii) une enqu\u00eate entomologique pour \u00e9tudier la dynamique spatio-temporelle des anoph\u00e8les vecteurs dans la province de Sisaket, iii) le d\u00e9veloppement d&rsquo;un nouveau test mol\u00e9culaire pour am\u00e9liorer la sp\u00e9cificit\u00e9 et la sensibilit\u00e9 dans l&rsquo;identification des esp\u00e8ces du complexe Anopheles dirus, iv) une enqu\u00eate s\u00e9rologique utilisant le biomarqueur gSG6 P1 pour mesurer l&rsquo;exposition aux piq\u00fbres de moustiques anoph\u00e8les, v) une enqu\u00eate en sciences sociales pour examiner les connaissances, les attitudes et les pratiques (CAP) parmi les populations locales dans les zones \u00e0 risque de transmission, et vi) une \u00e9valuation de r\u00e9pulsifs anti-moustiques en semi-champ et sur le terrain.<\/p>\n<p>Les r\u00e9sultats de ce projet ont montr\u00e9 qu&rsquo;An. dirus est le principal vecteur et ont confirm\u00e9 gr\u00e2ce au peptide gSG6 P1 que le risque de contracter le paludisme est \u00e9lev\u00e9 parmi les participants engag\u00e9s dans des activit\u00e9s dans les plantations d\u2019h\u00e9v\u00e9as et en for\u00eats. Par cons\u00e9quent, il est essentiel de donner la priorit\u00e9 \u00e0 des mesures de lutte antivectorielle efficaces et int\u00e9gr\u00e9es, ainsi qu&rsquo;\u00e0 l&rsquo;\u00e9ducation \u00e0 l&rsquo;\u00e9chelle locale, pour les groupes \u00e0 risque afin d&rsquo;\u00e9liminer le paludisme et d&#8217;emp\u00eacher sa r\u00e9apparition.<\/p>\n<p>\u00a0<\/p>\n<p><strong>Summary<\/strong><\/p>\n<p>To achieve the national goal of eliminating malaria, it is necessary to understand the risk of malaria transmission in the local context. This project aimed to identify the risk of malaria transmission in Sisaket Province, Thailand, which borders Cambodia and is historically recognized as a high malaria prevalence area.<\/p>\n<p>The thesis investigated several components: i) a literature review of malaria transmission and its progress after implementing control and elimination strategies in Thailand and at the local scale, ii) an entomological survey to investigate spatio-temporal dynamic of <em>Anopheles<\/em> vectors in the Sisaket Province, iii) the development of a new molecular assay to improve specificity and sensitivity in identifying species within the <em>Anopheles dirus<\/em> complex, iv) a serological survey using the gSG6 P1 biomarker to measure biting exposure from <em>Anopheles<\/em> mosquitoes, v) a social science survey to examine knowledge, attitudes and practices (KAP) among local people in areas at risk of transmission, and vi) an assessment of mosquito repellents at the semi-field and field trial levels.<\/p>\n<p>Overall, the data from this project showed that <em>An. dirus<\/em> is the main vector and it confirmed based on the gSG6 P1 peptide that the risk of acquiring malaria is high among participants engaged in activities in rubber and forest areas.
